Get a Library Card

Any resident of the White Oak Library District may have a library card card by completing the application form and furnishing proper identification. Proper identification is a picture ID verifying the person's name and address. If the picture ID does not also display a current address that proves residency in the District, a second form of ID is required, such as a utility bill, voter's card, credit card bill, bank statement, etc.

Until age 18, patrons must have a parent or legal guardian furnish proof of residency, and the minor applicant must be present when issuing the card.

The District will perform periodic address verification on all accounts. You may check your address to see if you are a resident in the White Oak Library District at the Will County Supervisor of Assessments website or the Will County Clerk website.

You may download a Library card application here in English, y descarga la applicación en español. Simply print the form, fill in the information, and bring it to the library at your convenience.
